How To Handle Ill-Fitting Zirconia Crowns

Zirconia crowns have become a popular choice in dental restorations due to their durability and versatility. However, achieving a perfect fit can be a challenge. Here’s a guide on identifying and addressing the causes of ill-fitting Zirconia crowns, along with solutions to ensure optimal results.

Causes of Ill-Fitting Zirconia Crowns

  • Rushed Production: Hastily made crowns can lead to inaccuracies in design and contouring, causing poor fit.

  • Inadequate Margins: Poorly defined margins allow saliva and bacteria to infiltrate, leading to decay and periodontal issues.

  • Improper Filling Process: Loose fillings can result in gaps, allowing entry of debris and bacteria.

  • Inaccurate Impressions: Errors in impression-taking can lead to incorrect data for crown fabrication.

  • Preparation Errors: Mistakes during tooth preparation can prevent the crown from fitting properly.

Causes of Ill-Fitting Zirconia Crowns

Fixing Ill-Fitting Zirconia Crowns

  • Adjust Cementing Process: Before cementing, check the fit of the crown in the patient’s mouth. Clean the teeth with pumice stone to enhance adhesion.

  • Clean Intaglio Surface: Thoroughly clean the inner surface of the crown to improve bonding. Proper cleaning ensures better adhesion of the cement.

  • Change Adhesive Technique: Apply adhesive, place the crown, and remove excess cement carefully to ensure a snug fit.

Removing Phosphate Groups

Removing Phosphate Groups

Removing phosphate groups can prevent ill-fitting issues:

  • Sandblasting: Use gentle sandblasting at low pressure to prepare the tooth surface.

  • Sodium Hypochlorite: Clean the crown’s intaglio with gauze soaked in a 5% sodium hypochlorite solution, rinse with water, and let it dry.

  • Ivoclean: Apply Ivoclean to the intaglio surface for 20 seconds, rinse, and let it dry naturally.

Addressing Facial-Lingual Problems

Facial-lingual fitting issues may arise due to digital imaging inaccuracies. To resolve this:

- Adjust Computer Settings: Use software to set appropriate margins and specify space between the fit surface and the prep site. This mimics the function of a die-spacer, preventing the crown from rocking facial-lingually.
